Developing Independence

Pathways to a Great Life

Life is a journey but you don’t have to do it alone. Using our Pathways Program, Midway Community Care can help you to achieve the life you want.

Midway is excited to provide a range of learning opportunities to support people to develop greater independence in the home, in the community or toward employment.

As part of our Pathways Program we use a person-centred approach focused on enhancing independence, confidence, self-esteem, interaction skills and life choices. The Pathways Program is designed to support people to achieve the life they want and to continually increase their personal potential. Our staff engage with individuals to learn what is important to them and we work together to develop a personalised plan. This plan is flexible to change, interests or choices.
